Below are the two service learning documents that need to be completed. If you have more than one service learning teacher, you will need to have EACH service learning teacher fill out a service learning contract and service learning evaluation form. Your Junior Achievement teacher, unless you also complete your service learning hours with him/her, is NOT to fill out these forms.
